For the safety of our staff and our clients, we reserve the right to cancel or reschedule service for anyone who might pose a health risk at this time. To aid us in that effort, we ask you to complete the following form before your appointment. If you are unable to confirm any of the following questions, please contact us immediately to discuss your situation.

I wish to participate in Yoga-Tastic 4 Kids Inc. suspension yoga program using the YogiGym®Suspension System. I hereby agree that the use of the YogiGym® Suspension System is voluntary and at my own risk. I understand that it involves a certain level of risk of injury, and that these risks and dangers exist while using the YogiGym® Suspension System. These risks may include physical and psychological injuries. As a condition of using the YogiGym® Suspension system, I accept the risks of using the YogiGym® Suspension System equipment and expressly agree to forever discharge, waive and release Yoga-Tastic 4 Kids Inc., its owners, agents, management, insurers,landlords, consultants and employees from any and all claims, demands, injuries, damages, actions, or courses of action, for all acts of active or passive negligence on the part of YogiGym® Suspension System or Yoga-Tastic 4 Kids Inc. on account of physical injury, mental anguish, death and/or property damage arising from any mishap, accident, loss, damage or injury I suffer or by others resulting from, connected with or caused by my use of the YogiGym® Suspension System, including, but not limited to, any injury resulting from defects or failure of any equipment or devices used in such services, facilities, and/or athletic/health equipment.

I declare and affirm that I am in good medical and physical condition and that the use of the YogiGym® Suspension Sling does not pose any danger to my health. I agree that I will not engage in behavior which may be hazardous, dangerous, or may interfere with the enjoyment of other persons participating in this program.

If despite the signing of this Waiver and Assumption of all Risk, a lawsuit is brought against YogiGym® or Yoga-Tastic 4 Kids Inc., its owners, agents, management, insurers, landlords, consultants or employees, I agree to pay for any and all court costs and attorney’s fees incurred as a result of such litigation. I understand this waiver and release is a material part of the consideration for me to use the YogiGym® Suspension System and participate in the activities of Yoga-Tastic 4 Kids Inc. As such, this is a full and final release applying to all unknown and unanticipated damages arising out of my use of the YogiGym® Suspension System, as well as to those now known or disclosed, and the undersigned waives all rights or benefits which the undersigned now has or in the future may have. I agree to abide by the policies of Yoga-Tastic 4 Kids Inc. and will not duplicate any portion of any programs or classes without the expressed written consent of Yoga-Tastic 4 Kids Inc.
